When you come to the office always bring your current insurance card and desired form of payment that will be collected prior to your visit. If you are paying with cash, please bring exact payment as we are unable to make change. We accept:

• Medicaid (Traditional, Texas Children's Health Plan CHIP and Star)
• Insurance
• You may opt to pay out of pocket and file for insurance reimbursement under out of network provider services, if we are not providers under your insurance plan
